Program Selection Form In order to help us plan for the reopening of our school, we need to know how your child will participate in instruction. A Program Selection Form may be completed online. Please complete this form by Friday, March 19 for each child in your household. If we do not receive a response from you, we will plan to continue providing online-only instruction for your child.
